Benefits of Personal Accident policy

Platinum cover policy for ladies:

  • Up to €10,000 for medical expenses.
  • Cover for consultations/operations in any public or private hospital of their choice.
  • €175 or 10% excess (whichever is the greater).
  • €250 for Physiotherapy.
  • €1500 for any emergency dental treatment required within 48 hours of the accident.
  • If the player holds a current Private Health insurance policy with any private health insurer such as VHI, Aviva, they must submit their claim to their Private Health Insurer first and any expenses that are not covered under that policy will be covered by O’Driscoll O’Neil.
  • €750 for any Loss of Earnings.

All players must submit a claim to the Social Welfare office to see if they are entitled to Injury Assistance while they are out of work. Once they receive confirmation they must forward this on to the O’Driscoll O’Neil office with their claim. If a player is entitled to Injury Assistance they must forward their weekly receipts of payment to the office.

Claims Procedure

When you sustain an injury please make sure you tell your manager immediately. Make sure he informs the club administrator and they obtain a claim form.
When you receive the claim form this MUST be completed by a doctor.
The form must also be signed by a club official or manager.
Submit claim to Private Medical Insurer (if any)
For Loss of Earnings claims, submit claim to Social Welfare.
Submit all docs to O’Driscoll O’Neil when completed.

Process and Summary of Cover & Incident Report Form


1. Aviva Claim form, this is completed by the player & hospital/Physio they
attend - the player completes the 1st page & 2nd page (MEDICAL
CERTIFICATE) is completed by the Hospital or Physio the player
attends.


**PLEASE NOTE ** IF A PLAYER HAS AN APPOINTMENT IN THE BEACON, THE
PLAYER SHOULD HAVE THE AVIVA CLAIM FORM COMPLETED BEFORE
ARRIVING TO SPEED UP THE PROCESS & €150.00 EXCESS PAYMENT FOR
THEIR 1ST APPOINTMENT ONLY (PER INJURY). THIS FORM IS THEN LEFT IN
RECEPTION OF ORTHOPEDICS– NO PLAYER SHOULD LEAVE THE BEACON
WITH THIS CLAIM FORM - I STRONGLY ADVISE ANY PLAYER ATTENDING
TAKES A PICTURE OF THE COMPLETED CLAIM FORM FOR THEIR OWN
RECORDS **


2. Incident report form – THIS NEEDS TO BE SENT TO ARACHAS
STRAIGHTAWAY FROM A CLUB OFFICAL TO START THE PROCESS
OF THE CLAIM,


** PLEASE NOTE ** WITHOUT ARACHAS RECEIVING THIS FORM NO CLAIM
CAN BE MADE AND ANY TREATMENT WILL BE HELD UP OR REFUSED.
In the event that the claim has been notified to Arachas more than 10 days after the date of injury, they
will require an explanation in writing as to the reason for the delay in notification. Insurers may reserve
their right to deal with any claims that are notified outside this time frame.
3. Any player going to A&E in the Beacon will need to pay all costs up
front and claim back through Arachas with the completed claim form &
Receipts.


4. PRIVATE HEALTH INSURANCE – ANY PLAYER WHO HAS A PRIVATE
HEALTH INSURANCE POLICY (VHI, IRISH LIFE OR LAYA) – OUR
POLICY WILL ONLY COVER THE COST OF MEDICAL TREATMENT
WHEN IRRECOVERABLE FROM THESE POLICIES SO BASICALLY THE
PLAYER WILL CLAIM FROM THEIR PRIVATE HEALTH INSURANCE
POLICY AND ANYTHING THAT CANNOT BE CLAIMED BACK THEN
GOES THROUGH THIS POLICY, I WILL ALSO LIKE TO STRESS THAT
A PRIVATE HEALTH INSURANCE POLICY WILL NOT PAY FOR LOSS
OF EARNINGS - OUR POLICY WILL UP TO €400 per week to a maximum
of 75% of your net weekly earnings including all other sources of
income.
